ABSTRACT

Objective:To evaluate the current situation of prevention and control of iodine deficiency disorders (IDD), and to describe the iodine nutrition level of pregnant women in key population in Zhejiang Province.Methods:In 2021, a cross-sectional survey on iodine nutrition status of the population was conducted in all 90 counties (cities, districts) in Zhejiang Province. Each county (city, district) was divided into five districts: east, west, south, north, and central. One township (street) was selected from each district, and 20 pregnant women were selected from each township (street) to collect edible salt samples and one random urine sample for salt and urinary iodine testing. The above indicators were stratified and analyzed according to urban, rural, coastal, inland, early pregnancy, mid pregnancy, and late pregnancy.Results:A total of 9 679 pregnant women's salt samples were tested, with a salt iodine content of (20.2 ± 9.2) mg/kg, an iodized salt coverage rate of 84.2% (8 153/9 679), and a qualified iodized salt consumption rate of 80.4% (7 785/9 679). Among them, the qualified iodized salt consumption rate of pregnant women in coastal areas was 69.4% (3 175/4 575), significantly lower than that in inland areas [90.3% (4 610/5 104)]; and the proportion of non iodized salt consumption in coastal areas [25.8% (1 181/4 575)] was significantly higher than inland areas [6.8% (345/5 104)]. A total of 9 679 pregnant women's urine samples were tested, with a median urinary iodine of 133.0 μg/L ( < 150 μg/L). In urban areas, the median urinary iodine (129.8 μg/L) was lower than that in rural areas (135.0 μg/L). Likewise, the median urinary iodine was 126.0 μg/L, lower than inland areas (140.3 μg/L). And as pregnancy progressed, the iodine nutrition level of pregnant women gradually decreased (the median urinary iodine: 137.0 μg/L in early pregnancy, 134.1 μg/L in mid pregnancy, and 129.4 μg/L in late pregnancy).Conclusion:In 2021, pregnant women in Zhejiang Province are in a state of iodine deficiency, and there is a greater risk in urban areas compared to rural areas and coastal areas compared to inland areas.

ABSTRACT

Objective@#To investigate the status of iodine nutrition and prevalence of goiter among children at ages of 8 to 10 years in Zhejiang Province in 2021, so as to provide insights into the management of iodine deficiency in children.@*Methods @#A total of 90 counties (districts) were sampled as field survey sites from 11 cities of Zhejiang Province using a multi-stage stratified random sampling method in 2021, and non-residential children at ages of 8 to 10 years in these sites were sampled as study subjects. Subjects' household edible salt samples and random urine samples were collected. The iodine content in salt and urinary iodine level were determined using the direct titration method and arsenic-cerium catalytic spectrophotometry for evaluation of iodine nutrition among children. In addition, the lateral lobe of the thyroid gland was measured using ultrasound, and the prevalence of goiter was estimated. @*Results@#A total of 19 363 children were recruited, including 9 710 male children and 9 653 female children, with a male to female ratio of 1.01︰1, and there were 10 704 urban children (55.28%) and 8 659 rural children (44.72%), 9 149 children living in coastal areas (47.25%) and 10 214 children in inland regions (52.75%). The mean iodine content was (19.79±9.25) mg/kg in salt, and the coverage of qualified iodized salt (15 252) was 78.77%. The coverage of qualified iodized salt was significantly lower in urban children than in rural children (76.70% vs. 81.21%; χ2=68.301, P<0.001), and was lower in children living in coastal regions than in inland regions (68.05% vs. 88.27%; χ2=1 270.769, P<0.001). The median urinary iodine concentration was 195.1 μg/L, and the household iodine content in salt correlated positively with urinary iodine concentration in children (rs=0.383, P<0.001). There were 2 885 children with iodine deficiency (14.90%), 7 137 children with adequate iodine (36.86%), 5 414 children with excessive iodine intake (27.96%), and 3 927 children with iodine overdose (20.28%), and the distribution of iodine nutrition in children varied significantly in regions (χ2=283.277, P<0.001) and gender (χ2=126.349, P<0.001). The prevalence of goiter was 2.45% among 7 195 children receiving ultrasound examinations, and a higher prevalence rate of goiter was detected in urban children than in rural children (2.76% vs. 2.00%; χ2=3.962, P=0.047). @*Conclusions @#The overall urinary iodine nutrition was adequate among children at ages of 8 to 10 years in Zhejiang Province in 2021, and the prevalence of goiter in children fell within the threshold defined in the criteria of elimination of iodine deficiency. However, the supervision of the iodized salt quality remains to be improved and iodine deficiency control remains to be reinforced.

ABSTRACT

Objective:To analyze the expression of the recombinant human phenylalanine-tRNA ligase beta subunit(FARSB)gene in hepatocellular carcinoma(HCC)and explore its association with clinicopathologic characteristics and prognosis.Methods:Data sets of hepatocellular carcinoma and paracancerous tissues were downloaded from the Cancer Genome Atlas(TCGA), and the data were analyzed using the R and Perl programming languages.Cox regression and the Kaplan-Meier method were used to analyze the relationship between clinicopathological characteristics and overall survival.Gene set enrichment analysis(GSEA)was used to predict the signal pathways involved in the regulation of FARSB.qPCR and Western blot were used to verify the expression level of FARSB in hepatocellular carcinoma and adjacent tissues.Results:FARSB was highly expressed in HCC and the prognosis of HCC patients with high FARSB expression was poor.The clinical stage, T stage, M stage and FARSB were significantly correlated with overall survival, while only high FARSB expression was an independent prognostic factor in HCC patients.Conclusions:High expression of FARSB indicates a poor prognosis in HCC patients and FARSB can be used as a marker for poor prognosis in HCC.

ABSTRACT

In recent years, the relationship between inflammation and the occurrence and development of cancer has attracted widespread attentions. As a pro-inflammatory factor, interleukin-6 (IL-6) plays a certain role in promoting cancer progression, and it is an important activating factor of signal transducer and activator of transcription 3 (STAT3). Many studies have shown that continuous activation of the IL-6 and STAT3 signaling pathway can induce the abnormal expression of oncogenes related to cell proliferation, apoptosis and differentiation in colorectal cancer. This article reviews the role of IL-6 and STAT3 signaling pathway in the occurrence and development of colorectal cancer, as well as the clinical value of this signaling pathway in the prognostic evaluation and treatment of colorectal cancer, in order to provide certain theoretical basis for the disease monitoring, prognostic evaluation and treatment of colorectal cancer.

Objective:In order to solve the problem that it is inconvenient and easy to lose the transfusion rod when nurses use the flat car to transport the patients who need transfusion or transfusion at the same time with continuous washing by installing the recyclable transfusion rod on the original medical flat car.Methods:A recyclable infusion rod was installed in the medical flat car. According to the sequence of operation time, 160 patients in the operation room of our hospital were divided into the experimental group and the control group, 80 cases in each group. The experimental group used the medical flat car equipped with the recyclable infusion pole, and the control group used the ordinary flat car and the movable flagpole infusion pole. When the patients need to be transported after the operation, the time when the nurses go out from the operating room to pick up the flat car and the rate of the remaining transfusion rod after the patients are transported are compared to evaluate the application effect.Results:The time of taking flat car of the nurses in the experimental group was (63.34±4.72) seconds, which was significantly lower than (83.6±13.82) seconds of the control group ( t value was -29.89, P < 0.01). The rate of remaining infusion pole of the nurses in the control group was 6.25% (5/80), and that in the experimental group was 0, with statistical significance ( χ2 value was 5.16, P<0.05). Conclusion:The installation of recyclable infusion pole on the medical flat car can keep the cleanness of the infusion pole and reduce the incidence of hospital infection. When using the flat car, it can also reduce the time of taking the infusion pole again, reduce the workload of the nursing staff, put an end to the loss of the infusion pole and improve the working efficiency of the nursing staff.

Introducción: Como problema de la práctica de la profesión, la comorbilidad implica mayor complejidad en el plano del diagnóstico, el tratamiento, la creación de protocolos de atención y en la investigación. Es necesario identificar la comorbilidad del paciente hipertenso atendido en salas de medicina interna y su ordenamiento en mecanismos etiopatogénicos, fisiopatológicos y terapéuticos. Objetivo: Identificar la magnitud de la comorbilidad en pacientes con diagnóstico de hipertensión arterial ingresados en salas de Medicina interna. Material y Métodos: Se realizó una investigación descriptiva y prospectiva de 144 pacientes hipertensos. Se recogió la información necesaria a partir de la entrevista y examen clínico de los enfermos, las historias clínicas y la discusión con los médicos de asistencia. Los datos obtenidos se registraron en la planilla confeccionada al efecto y luego fueron vertidos en la base de datos del paquete estadístico SPSS versión 11.5. Resultados: Las comorbilidades más frecuentes fueron la diabetes mellitus tipo 2 en el 72,9 por ciento; la cardiopatía isquémica 54,9 por ciento y la enfermedad cerebrovascular 36,1 por ciento-. Conclusiones: La magnitud de la comorbilidad en los hipertensos que ingresan en salas de Medicina interna tiene como asociación etiopatogénica más importante a la diabetes mellitus tipo 2 y a las complicaciones ateroscleróticas derivadas de ambas entidades como las asociaciones fisiopatológicas más relevantes(AU)


Introduction: Comorbidity, as a problem in the practice of the medical profession, implies greater complexity in terms of diagnosis, treatment, creation of care protocols, and research. It is necessary to identify comorbidity in the hypertensive patient treated in internal medicine wards and establish etiopathogenic, pathophysiological, and therapeutic mechanisms. Objective: To identify the magnitude of comorbidity in patients with arterial hypertension admitted to the Internal Medicine wards. Material and Methods: A descriptive and prospective research was carried out on 144 hypertensive patients. The necessary information was collected from medical interviews, patients´ clinical exams, clinical histories, and case discussions with the doctors that were providing medical care to these patients. The data obtained were recorded in the database of Statistical Package SPSS software version 11.5. Results: The most frequent comorbidities were type 2 diabetes mellitus in a 72.9 percent; ischemic heart disease in a 54.9 percent, and cerebrovascular disease in a 36.1 percent. Conclusions: The magnitude of comorbidity in hypertensive patients admitted to Internal Medicine Wards recognizes Type 2 Diabetes Mellitus and atherosclerotic complications derived from both entities, as the most relevant pathophysiological associations(AU)

Objective@#To improve clinical management of maternal and neonatal listeriosis through analyzing the clinical characteristics and antibiotic treatment.@*Methods@#A retrospective analysis of 26 cases of listeriosis, including their demographic and clinical features, was conducted, involving 16 pregnant women from Civil Aviation General Hospital, Xiamen Humanity Hospital, Tongzhou Maternal and Child Health Hospital of Beijing, Beijing Tiantan Hospital, Tangshan Maternal and Child Health Hospital, the Fourth Hospital of Tianshui City from October, 2011 to May, 2018, and 10 newborns from the Fourth Hospital of Tianshui City, Tangshan Maternal and Child Health Hospital, Zhengzhou Children's Hospital from February, 2016 to April 2018. Descriptive methods were used for data analysis.@*Results@#(1) Among the 16 gravidas, one, five and 10 developed the infection in the 1st, 2nd and 3rd trimester of pregnancy, respectively, and eight had pregnancy complications. Furthermore, all of them developed fever [(38.9±0.5) ℃]. Symptoms such as cough, nasal congestion, runny nose, sore throat, dizziness, headache and other flu-like symptoms were observed in six cases. Gastrointestinal symptoms and flu-like symptoms were presented in four. Fetal distress, tachycardia and decreased fetal movement occurred in 11 cases. Elevated C-reactive protein and white blood cell count were detected in 16 and 14, respectively. Eight underwent placental pathological examination which shown various degrees of pathological changes, including neutrophil infiltration, acute chorioamnionitis and inflammatory necrosis. The main empirical antibiotic treatment for the 16 patients was cephalosporins and only four covered Listeria monocytogenes. Only two delivered at term, while the others (14/16) ended in miscarriage, premature delivery or stillbirth. (2) Among the 10 newborns with listeriosis, there were eight early-onset infections and two late-onset infections. All of them were febrile [(38.6±0.6)℃]. Six had cyanosis, groaning, foaming and three concave sign; five showed shortness of breath; meningitis and skin rash were found in one, respectively. All had elevated white blood cell and C-reactive protein. Six received antibiotics covering Listeria monocytogenes during the initial empirical treatment. Four were treated with cephalosporins alone, one of which died after the treatment was withdrawn. One was cured by initial treatment with meropenem alone, while eight recovered after adjustment of treatment with ampicillin, penicillin, meropenem, vancomycin alone, or meropenem combined with ampicillin or vancomycin. (3) The isolates that were susceptible to penicillin, ampicillin and meropenem accounted for 96.1% (25/26) all together and 88.5% (23/26) were susceptible to compound sulfamethoxazole.@*Conclusions@#There is no specific clinical manifestations of maternal or neonatal listeriosis. Maternal listeriosis is often characterized by acute onset and high incidence of adverse pregnancy outcomes.

ABSTRACT

Introducción: La hipertensión arterial es un síndrome que tiene como centro un signo obtenido mediante una medición puntual y repetida. Existen diversas variables que resultan de la medición de la tensión arterial que, a su vez es afectada por muy diversos factores. Objetivo: Fundamentar el manejo de la hipertensión arterial como variable de investigación. Material y Método. Se realizó una revisión bibliográfica en fuentes de información disponibles en la Biblioteca Virtual de Salud de la red telemática infomed, entre ellas, las bases de datos SciELO, Pubmed/Medline, Cumed, Lilacs, así como el Google académico. Finalmente se seleccionaron para nuestro estudio un total de 39 referencias. Desarrollo: Las complejidades de la hipertensión como variable se derivan de las variadas formas de clasificarla, medirla y graficarla. También del número de mediciones y el momento evolutivo de la enfermedad, de sus causas y asociaciones. Conclusiones: La investigación sobre hipertensión arterial exige que en el diseño se prevea que subvariables internas y externas a la HTA serán valoradas. Así como la validez y confiabilidad de las utilizadas y tomar esos elementos en cuenta durante la discusión de sus resultados para que las conclusiones y recomendaciones sean coherentes(AU)


Introduction: Arterial hypertension is a syndrome which has, as its center, a sign which is obtained by means of a punctual and repeated measurement. There are several variables resulting from the measurement of blood pressure, which in turn, is affected by multiple factors. Objective: To establish the management of arterial hypertension as a research variable. Material and Methods: A bibliographic review was carried out thorough the search for information on different available sources from the Virtual Health Library of the Cuban National Health Care Network and Portal (INFOMED). Among them, databases such as SciELO, Pubmed/Medline, Cumed, Lilacs, as well as Google Scholar were accessed. Finally, a total of 39 references were selected for our study. Development: The complexities of hypertension as a variable are derived from the different forms of classifying, measuring, and graphing it; and also from the number of measurements and the moment during the follow-up of the disease, its causes, and associations. Conclusions: Research on arterial hypertension demands that, in its design, the assessment of its internal and external sub-variables, as well as the validity and reliability of the ones used, should be foreseen to take all these elements into account during the discussion of their results to in order that the conclusions and recommendations can be coheren(AU)

ABSTRACT

Introducción: el reingreso de un paciente diabético es un indicador en el que interaccionan la calidad de la asistencia sanitaria, las características particulares del paciente y quienes le prestan ayuda. Implica también incrementos de complicaciones, mortalidad y costos para el sistema de salud y el paciente. Los reingresos más tardíos pueden obedecer a problemas derivados de la evolución de las enfermedades en los pacientes, como pueden ser la comorbilidad y la gravedad de la enfermedad. También a los cuidados recibidos fuera del hospital. Objetivo: identificar factores asociados al reingreso de pacientes diabéticos con alta comorbilidad. Método: estudio descriptivo de serie de casos, prospectivo, con 110 pacientes ingresados en una sala de Medicina Interna de un hospital en La Habana. Como variable resultado se consideró el reingreso hospitalario en tres meses de seguimiento. Resultados: el 35,5 por ciento de los casos reingresaron en menos de tres meses. La edad promedio de las 61 mujeres fue de 70,38 años y de los 49 hombres 66,19 años; las comorbilidades más frecuentes fueron la hipertensión 95,5 por ciento, cardiopatía isquémica 61,8 por ciento y obesidad central 56,4 por ciento; el puntaje del índice de Charlson fue de 4,41 en los casos reingresados y 3,54 en los no reingresados. Conclusiones: la alta frecuencia de reingresos en la serie se asocia a características que definen a los diabéticos en estudio como adultos mayores con altos índices de comorbilidades dependientes, fundamentalmente, de la presencia de complicaciones cardiovasculares de la diabetes asociada a hipertensión arterial(AU)


Introduction: The re-entry of a diabetic patient is an indicator in which the quality of health care, the particular characteristics of the patient and those who help them interact. It also implies increases in complications, mortality and costs for the health system and the patient. Later re-admissions may be due to problems derived from the evolution of diseases in patients, such as comorbidity and the severity of the disease, and also to the care received outside the hospital. Objective: to identify factors associated with the re-entry of diabetic patients with high comorbidity. Methods: This is a descriptive cases series study, prospective, with 110 patients admitted to the Internal Medicine ward of a hospital in Havana. As a result, variable, the hospital re-admission was considered in three months of follow-up. Results: 35.5 percent of the cases re-admitted in less than three months. Sixty-one were women and the average age was 70.38 years. Forty-nine were men and the average age was 66.19 years. The most frequent comorbidities were hypertension 95.5 percent, ischemic heart disease 61.8 percent and central obesity 56.4 percent. The score in Charlson index was 4.41 in the re-admitted cases and 3.54 in the non-re-admitted. Conclusions: The high frequency of readmissions in this series is associated with characteristics that define diabetics in the study as elderlies with high rates of dependent comorbidities, mainly, the presence of cardiovascular complications of diabetes associated with hypertension(AU)

ABSTRACT

Objective@#To study the efficacy of 1 mg·kg-1·d-1 and 2.0 mg·kg-1·d-1 of propranolol in the treatment of infantile hemangiomas, so as to provide an ideal dosage for clinical treatment.@*Methods@#From September 2015 to October 2016, there were 89 patients in accordance with the inclusion criteria of infantile hemangiomas. According to randomized and controlled principle, the patients were assigned to receive two propranolol regimens, Group A(n=45): propranolol at a dose of 1 mg·kg-1·d-1; Group B(n=44): propranolol at a dose of 2 mg·kg-1·d-1. 1 or 2 mg of propranolol base per kilogram per day, divided into two doses.The first dose was taken at 9, next at 15.The effective rate, cure rate and adverse effect rate were compared at the six months after treatment. The duration of the treatment was compared after 12 months′ treatment, and the recurrence rate was compared at the three months after being cured.The data were statistically processed with SPSS 22.0. The sample rate was compared with χ2 test, and the data were compared with t test. P<0.05 was statistically significant difference.@*Results@#Group A: Treatment for 6 months, 45 children were evaluated as follows: the effective rate was 91.1%(41/45), the cure rate was 60%(27/45). The incidence of adverse reactions was 13.3%(6/45). The cure rate of group A at 12 months was 86.7%(39/45). The duration of treatment was 7.6±2.7 months.The recurrence rate was 9.5%(4/42). Group B: Treatment for 6 months, 44 children were evaluated as follows: the effective rate was 90.9%(40/44), the cure rate was 72.7%(32/44). The incidence of adverse reactions was 15.9%(7/44). The cure rate of group B at 12 months was 88.6%(39/44). The duration of treatment was 6.2±1.9 months. The recurrence rate was 11.9%(5/42). The effective rate of the two groups at 6 months was not statistically significant(χ2=2.583, P=0.461). The cure rate of the two groups at 6 months was statistically significant(χ2=8.339, P=0.004). The incidence of adverse effects was not statistically significant(χ2=0.118, P=0.731). The cure rate of the two groups at 12 month was not statistically significant(χ2=0.080, P=0.778). The duration of treatment between the groups was statistically significant (t=0.290, P=0.009). The recurrence rate of the two groups after 3 months withdrawal was no statistical difference(χ2=0.124, P=0.724).@*Conclusions@#The propranolol doses at 1 mg·kg-1·d-1 and 2 mg·kg-1·d-1 in the treatment of infantile hemangiomas are safe and effective. The propranolol dose at 1 mg·kg-1·d-1 doesn′t decrease the effective and cure rate, not increase the recurrence rate in infantile hemangiomas. Low dose at 1 mg·kg-1·d-1 can be used as a common dose of propranolol in infantile hemangiomas.

ABSTRACT

<p><b>OBJECTIVE</b>To observe the clinical therapeutic effects on severe acute pancreatitis (SAP) treated with electroacupuncture (EA), sparse-dense wave and 2 Hz/15 Hz, at Dachangshu (BL 25) and Shangjuxu (ST 37) assisting ulinastetin and explore the effective therapeutic method for SAP.</p><p><b>METHODS</b>A total of 120 patients of SAP were randomized into an observation group and a control group, 60 cases in each one. In the control group, the routine western medicine was adopted with the intravenous drip with ulinastatin. In the observation group, on the basis of the treatment as the control group, EA was applied at Dachangshu (BL 25) and Shangjuxu (ST 37). The treatment was given once every day, 20 min each time. The treatment was 2 weeks in the two groups. The recovery time of defecation, the recovery time of bowel sound, the remission time of abdominal pain and the hospitalization time were recorded in the patients of the two groups. The changes in the tumor necrosis factor alpha (TNF-α), interleukin-6 (IL-6) and amylase, as well as the scores in the acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) were compared in the patients of the two groups before treatment, and on the 3rd, 8th and 14th days of treatment. The therapeutic effects, the cases of surgical transfer or the cases of the transfer of intensive care unit (ICU) were compared between the two groups.</p><p><b>RESULTS</b>The recovery time of defecation, the recovery time of bowel sound and the remission time of abdominal pain, as well as the hospitalization time in the observation group were shorter than those in the control group (all<0.05). The levels of TNF-α, IL-6 and amylase, as well as the scores of APACHE Ⅱ on the 3rd, 8th and 14th days of treatment were all reduced as compared with those before treatment in the two groups (all<0.05). The results in the observation group were lower than those in the control group (all<0.05). The total effective rate was 95.0% (57/60) in the observation group, better than 81.7% (49/60) in the control group (<0.05). There were 1 case of surgical transfer and 0 case in ICU transfer in the observation group, 3 cases of surgical transfer and 2 cases in ICU transfer in the control group, indicating the significant differences between the two groups (both<0.05). .</p><p><b>CONCLUSION</b>EA at Dachangshu (BL 25) and Shangjuxu (ST 37) displays the satisfactory accessory therapeutic effects on SAP treated with ulinastatin.</p>

ABSTRACT

<p><b>OBJECTIVE</b>On the basic treatment, to observed the effect difference between acupuncture at Zhiyang (GV 9) points combined with pantoprazole and simple pantoprazole for gastroesophageal reflux cough with damp-heat type.</p><p><b>METHODS</b>A total of 102 patients were randomly assigned into an observation group and a control group, 51 cases in each group. The patients in the two groups were given domperidone tablets (10 mg each time, 3 times a day). The patients in the control group were treated with pantoprazole capsule (40 mg each time, once a day). On the basis of the control group, the patients in the observation group were treated with Zhiyang (GV 9) points, once a day, 5 times a week. All the treatment was given for 8 weeks. The indexes were observed before and after treatment, including cough symptom at daytime and nighttime scores, TCM symptom (heartburn, acid regurgitation, chest pain, epigastric pain, throat discomfort) scores and quality of life scores of cough questionnaire in Leicester (physiological, psychological and social aspects). The clinical effects were compared.</p><p><b>RESULTS</b>After treatment, the daytime and nighttime cough scores, TCM symptom (heartburn, acid regurgitation, chest pain, epigastric pain, throat discomfort) scores were lower, and the quality of life scores of cough questionnaire in Leicester (physiological, psychological and social aspects) were higher than those before treatment in the two groups (all <0.05), with better results in the observation group (all <0.05). The total effective rate of the observation group was 94.1% (48/51), which was better than 80.4% (41/51) of the control group (<0.05).</p><p><b>CONCLUSION</b>On the basic treatment, acupuncture at Zhiyang (GV 9) points combined with pantoprazole can improve TCM symptoms, such as cough, of the patients with gastroesophageal reflux cough with damp-heat type, and improve their quality of life.</p>

Introducción: La comorbilidad es un fenómeno que complejiza el cumplimiento de las funciones del médico generalista en la atención a pacientes en salas de medicina interna. Objetivo: Fundamentar las bases de la comorbilidad como variable, en tanto se define en lo conceptual, lo operacional y relaciones esenciales internas y externas con el enfoque en sistema de las funciones del médico clínico en las salas de Medicina Interna de los hospitales. Material y Métodos: Se realizó una revisión bibliográfica que abordasen la comorbilidad y que facilitaran cumplir el objetivo de la investigación. Desarrollo: En el plano conceptual se define la comorbilidad como la asociación de enfermedades donde una tiene carácter protagonista Se presenta la definición conceptual de la comorbilidad. La relación entre enfermedades comórbidas puede estar explicada por asociaciones de clara dependencia patogénica o por coincidencia en el paciente sin relación patogénica directa. Existen numerosos y diversos instrumentos para operacionalizarla disponibles para su aplicación en investigación y la práctica clínica. Conclusiones: Las relaciones entre las diferentes enfermedades son multilaterales y requieren de una interpretación etiopatogénica y fisiopatológica profunda para ser comprendida y analizada en el cumplimiento de las funciones asistenciales, investigativa, docentes y administrativas del médico en salas de Medicina Interna(AU)


Introduction: The comorbidity is a phenomenon that complicates general doctor´s fulfillment of their functions regarding the care of patients at Internal Medicine Wards. Objective: To base the bases of comorbidity as a variable, insofar it is defined in the conceptual, the operational and essential internal-external relationships with a systemic approach of the functions of the clinical physician in the hospital´s Internal Medicine wards. Material and Methods: A bibliographic revision that take into account comorbidity and allow to satisfy the objective was performed. Development: Conceptually, comorbidity is defined as the association of diseases in which one has a leading role character. The conceptual definition of comorbidity is presented. The relationship between comorbid diseases can be explained by associations of a clear pathogenic dependence or by coincidence in the patient without direct pathogenic relationship. There are numerous and diverse instruments available to make it operable for its application in research and clinical practice. Conclusions: The relations between different diseases are multilateral and need a deep etiopathogenic and fisiopathological interpretation to be understood and examined in the fulfillment of its asitencials, investigative, teaching and administrative functions of the doctor at Internal Medicine wards(AU)

ABSTRACT

OBJECTIVE:To investigate the effects of plasma exchange (PE) combined with intensive insulin glycemic control on related indexes of patients with HLP.METHODS:A total of 120 HLP patients were randomly divided into control group (60 cases) and observation group (60 cases).Based on routine treatment,2 groups were given PE treatment for 3 times+Low molecular weight heparin calcium for injection 5 000 U subcutaneously+Methylprednisolone sodium succinate for injection 40 mg intravenously+Calcium gluconate injection 2 g intravenously.Based on it,Insulin injection was pumped to random blood glucose <12 mmol/L with a micro-infusion pump in control group when random blood glucose concentration was equal to or more than 12 mmol/ L.When random blood glucose of patients in observation group were higher than 8.3 mmol/L,Insulin injection was pumped with a micro-infusion pump for a week and random blood glucose concentration was maintained at 6.1-8.3 mmol/L.Plasma levels of triglyceride,PCT,WBC count,APACHE Ⅱ score,Balthazar CT score,length of hospitalization stay in ICU and the occurrence of ADR were observed in 2 groups before and after treatment.RESULTS:After treatment,plasma levels of triglyceride,amylase and PCT,WBC count,APACHE Ⅱ score and Balthazar CT score in 2 groups were significantly lower than before treatment,and the observation group was significantly lower than the control group,with statistical significance(P<0.05).Length of hospitalization stay in ICU observation group was significantly shorter than control group,with statistical significance(P<0.05).There was no statistical significance in the incidence of ADR between 2 groups(P>0.05).CONCLUSIONS:PE combined with intensive insulin glycemic control can effectively reduce the level of plasma triglyceride,relieve inflammatory reaction and improve prognosis without increasing the occurrence of ADR.

Adipose tissue,derived from the mesoderm, consists of adipocytes and stromal vascular fraction.Stromal vascular fraction is a cell population that has not been fully identified, which contains a variety of stem/progenitor cells.Recent studies have indicated that adipose tissue contains a certain amount of hematopoietic stem/progenitor cells to maintain its hematopoietic regeneration potential,but the specific mechanism of regulation is not clear yet.This review is mainly about the research progress of adipose tissue and its relationship with bone marrow microenvironment, providing a new idea for understanding the hematopoietic regeneration potential of adipose tissue.

ABSTRACT

To study the regulation effect of Sheng-mai San (SMS)on intestinal dysbacteriosis,ceftriaxone sodium was used to induce intestinal dysbacteriosis in vitro,the bacteria were cultured in the selective medium and enu-merated by plat counting method,and the short chain fatty acid were also analyzed .The results showed that SMS (16.5,11.2,5.5 mg/mL)could significantly inhibit the proliferation of potentially harmful bacteria such as Enterococci,Enterobacteriaceae and promote the proliferation of Lactobacillus and Clostridum.Compared with the model group,the proliferation of Enterococci and Enterobacteriaceae was inhibited by 24.9% and 19.1%,while the proliferation of Lactobacillus and Clostridum was increased by 22.3% and 25.8% respectively in the middle-concentration of SMS group (11.2 mg/mL)at 24 h.Meanwhile,SMS in different concentration significantly increase the accumulation of acetic acid and propionic acid generated by gut microbes which could significantly inhibit the proliferation of harmful bacteria.And SMS showed prebiotic effects on intestinal microflora imbalance induced by antibiotics in vitro.

ABSTRACT

OBJECTIVE:To establish the method for the determination of mildronate in human plasma and urine,and to study the pharmacokinetic characteristics in healthy volunteers. METHODS:After precipitating plasma and urine sample,LC-MS/MS method was adopted. Dikma Diamonsil C18 column was used with mobile phase consisted of methanol-water(containing 0.2% for-mic acid,0.3% ammonium acetate)(31∶69,V/V)at the flow rate of 0.6 ml/min. ESI was adopted in MRM mode,by using nega-tive ion. The ion for quantitative analysis were m/z 147.10→58.20 (mildronate) and m/z 152.00→110.10 (internal standard,acet-aminophen). The pharmacokinetic parameters of mildronate with single administration and multiple administration were calculated by using DAS 2.1 software and compared. RESULTS:The linear range of mildronate in plasma were 0.02-20 ng/ml(r=0.999 3) and in urine were 0.05-40 ng/ml(r=0.998 2). The lowest limits of quantitation were 0.02 and 0.05 ng/ml. Precision and recovery met the requirements of biological specimen determination,and endogenous impurities hadn’t effect on the determination. The main pharmacokinetics parameters of low-dose,medium-dose and low-dose(250,500,750 mg)of mildronate in plasma with single ad-ministration were as follows:t1/2 were(3.39±0.81),(5.52±0.57)and(5.32±0.96)h;tmax were(0.80±0.45),(1.38±0.43)and (1.10±0.36)h;cmax were(4.17±1.46),(8.08±1.04)and(15.04±1.86)ng/ml;AUC0-36 h were(24.55±5.81),(45.50±7.07)and (85.60 ± 13.09)ng·h/ml. In the dose range,cmax,AUC0-36 h h had a linear relationship with dose (R2 were 0.974 5 and 0.968 3). The main pharmacokinetic parameters of low-dose of mildronate with multiple administration after keeping stable were as follows:cmin was(0.28 ± 0.10)ng/ml;AUCs was(38.78 ± 4.18)ng·h/ml;cs was(1.62 ± 0.17)ng/ml;DF was(3.81 ± 1.14);t1/2 was(6.17 ± 1.46)h;tmax was(1.20 ± 0.33)h;cmax was(6.46 ± 1.96)ng/ml;AUC0-36 h was(40.33 ± 4.65)ng·h/ml;accumulation factor of cmax and AUC were(1.73±0.90)and(1.64±0.40). Compared with single administration,t1/2,cmax and AUC of mildronate with multiple admin-istration after keeping stable all changed,and tmax had no signifi-cant difference. After single administration,26 h accumulative excretion rate of those groups were (0.004 009 ± 0.001 1)%, (0.004 026±0.001 01)% and(0.003 858±0.000 68)% respec-tively. CONCLUSIONS:Established method is sensitive,accurate and specific,and suitable for the determination of mildronate concentration in human plasma and urine and pharmacokinetics study. Mildronate capsule shows certain accumulation effect in healthy volunteers,and linear pharmacokinetic characteristics.

ABSTRACT

The statement on the depth of needle insertion is collected from Neiiing (nternal Classic) and Nanjing (Classic of Medical Problems) and the relationship of it is discussed with meridians, diseases, symptoms, location of sickness, qi and blood changes in human body, time, layers of needle insertion, reinforcing and reducing of acupuncture, etc. The factors are explored for the determination of the puncture depth, which provides the important guidance for the depth of insertion, needling sensation, reinforcing and reducing manipulations, acupuncture effect and the improvement of acupuncture efficacy. The complex and mystery of acupuncture technique focus on the control of insertion depth. In clinical practice, the increase of needle insertion depth may not induce the equal increase of efficacy. The depth of needle insertion is rather relevant with stimulation at the effective acupoint in the effective layer.

ABSTRACT

Objective To observe the therapeutic efficacy of acupuncture at the thirteen ghost points in treating postpartum depression and its effect on the quality of life. Methods Sixty patients with postpartum depression were randomized into a treatment group and a control group, 30 in each group. The treatment group was intervened by acupuncture at the thirteen ghost points, while the control group was by oral administration of Fluoxetine. The Hamilton Rating Scale for Depression (HAMD) and the Short-Form (36) Health Survey (SF-36) were compared before and after intervention. Results The HMAD score was significantly changed in both groups after intervention (P<0.05). There was a significant difference in comparing the HAMD score between the two groups after intervention (P<0.05). There were significant differences in comparing the post-treatment bodily pain (BP), general health (GH), vitality (VT), social functioning (SF), mental health (MH), and health transition (HT) between the two groups (P<0.05). Conclusion Acupuncture at the thirteen ghost points can effectively improve the postpartum depression and the quality of life.
